Band Trip Medical Information
|
| If your child takes prescription or over the
counter medication, we will need you to complete some medical forms prior to
the Washington DC Trip. Connecticut State Law requires a written order from an MD, DDS, OD, PA, or APRN and a parent's or guardian's authorization for both prescription and non-prescription medications. All medications shall be delivered to Vicki Pehmoeller by the parent, guardian, or other responsible adult. The medication must be stored in the original labeled container as dispensed from the pharmacy. The forms need to be completed, signed and presented to Vicki Pehmoeller prior to the trip so that the school health office can approve them. Forms will be collected during the mandatory parent's meeting coming up in April The following forms need to be completed: - Authorization for Medication by School Personnel form - Authorization for Medication by Volunteer Nurse - Authorization of a Parent or Guardian for the Administration of Ibuprofen or Aspirin Substitute Containing Acetaminophen If applicable, this form as well: - Metered Dose Inhaler (MDI) Self-Administration form for students in Grades 6 - 12 Please keep in mind that the students will be traveling a long distance on the bus and participating in very busy and long days. Without these forms, the nurses cannot give your child ibuprofen or other pain reliever. We encourage you to talk with your child about this. The forms were emailed to those subscribing to the marching band email blast.
