bullet Band Banquet Form (2011/2012)
 Banquet reservation form for this year's band banquet is available here (Monday November 15).
bullet Band Backers Membership Form (2011/2012)
 Support the music programs at SHS by becoming a band backer!  Your donation will help offset the costs of providing a superb music program for our students.
bullet Medical Treatment Authorization Form (2011/2012)
 Should an emergency situation arise, parents/guardians need to give permission to the Band Director to obtain proper medical aid, including hospitalization, for their son/daughter. This form needs to be filled out and returned to the band director during Band Camp.
bullet Meal Plan  (2011/2012)
 Coming Soon...The meal program was started in an effort to provide all Marching Band Members an opportunity to have a solid, well-balanced meal prior to each performance.
