This page will let you download various forms needed by the band program:

bullet Band Backers Membership Form (2008-09)
 Support the music programs at SHS by becoming a band backer!  Your donation will help offset the costs of providing a superb music program for our students.
bullet Medical Treatment Authorization Form (2008-09)
 Should an emergency situation arise, parents/guardians need to give permission to the Band Director to obtain proper medical aid, including hospitalization, for their son/daughter. This form needs to be filled out and returned to the band director during Band Camp.
bullet Meal Plan Brochure | Meal Plan Menu (2007-08)...coming soon
 The meal program was started in an effort to provide all Marching Band Members an opportunity to have a solid, well-balanced meal prior to each performance.

bullet Mandatory Uniform Fee (2008-09)
Costs associated with color guard uniforms and cleaning instrumental uniforms for the season.  Questions on the uniform fee can be directed to Ken Leclair.
